UnityPoint Health Medication History Pharmacy Technician in Bettendorf, Iowa

Under supervision of a licensed Pharmacist, the Medication History Pharmacy Technician is responsible for completing the most accurate current medication list possible on newly admitted inpatients through information obtained from the patient, family, pharmacy, physician office, or other facility as required. Inputting correct home medication information, dose, frequency, time of last dose, primary pharmacy, and allergy information into the electronic medical record. Has excellent knowledge of brand and generic medication names, common dosages and dosage forms and frequency of administration of each medication. Performs duties of Pharmacy Technician I and/or Pharmacy Technician II positions. Performs back-up Pharmacy buyer/purchasing position.

Medication History and Analysis

·Interview newly admitted patient or patient’s family to obtain the most accurate current medication list possible.

·Contact patient’s family, nursing facility, pharmacy, and/or physician office for medication clarifications as needed.

·Obtain indications for PRN medications and actual frequency of use of PRN medication.

·Prompt patient for additional medications by specifically suggesting over the counter (OTC) medications including herbals, vitamins, pain relievers, cold products, patches, lotions & creams, eye drops, sprays and inhalers.

·Determine if patient was on recent antibiotics prior to admission.

·Input most accurate current medication list possible into electronic health record.

·Update patient’s primary pharmacy and other pharmacies the patient may use in the electronic health record.

·Obtain medication allergy history and related reactions to the medication and update electronic health record.

·Identify medication errors on patient medication list and communicate findings to Pharmacist.

·Effectively able to use Microsoft Office products.

·Demonstrate effective interpersonal skills in communicating with patients, families, and other health care professionals.

·Assist with program improvement through quality assurance and identify processes needing adjustment.

·Assist Medication Reconciliation Supervisor in metric tracking and reporting.

·Assist Medication Reconciliation Supervisor in educating nurses, providers, students and Pharmacy staff on medication reconciliation.

Medication Preparation, Distribution, and Purchasing

·Prepare orders for Pharmacist verification.

·Perform non-sterile and sterile compounding.

·Receive, deliver and stock medications throughout the hospital.

·Provide backup for pharmacy buyer/purchasing as directed.

·Understand 340B program and compliance.

  • Completes all annual education and competency requirements within the calendar year.

  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Education:

High School Diploma or Equivalent

Experience:

2 years’ experience as a Pharmacy Technician

2 years of retail pharmacy experience as a Pharmacy Technician
