Department: iiiiiiiiiiii Nursingiiiiiiiiii Supervisor: ii iiiiiiiiiii Director of Nursing : Shift: iiiiiiiiiiiii iiiiiiiiiii Day, Evening, Night and Other ShiftsStatus: iiiiiiiiiiiiiiiiiiiiii Non-ExemptEDUCATION: High school graduate or GED preferred.i All applicants must be at least 16 years old.i Completion of a seventy-five-hour nurse aide course is required.i Universal Workers must be able to read, write and follow written and oral instructions in English.i Failure to maintain certification as a nursing assistant will result in termination.i Medication Aide certification is required.iiiii WORK EXPERIENCE:Previous experience as a universal worker in assisted living, as a certified nurse aide in a long-term care setting or as a home health aide is desired.CONTINUING EDUCATION REQUIREMENT: All Universal Workers are required to complete 12 hours of continuing education on an annual basis.i Failure to complete the twelve-hour requirement each year or any other local, state or federal requirement may result in termination.i GENERAL JOB SUMMARY:A Universal Worker is responsible for providing personal care and other assistance assigned.i Universal Workers may be called upon to help residents in and out of bed, and with dressing, grooming, personal hygiene, toileting, feeding and bathing. Additionally, Universal Workers will be requested to help with housekeeping and laundry tasks, stocking supplies, transporting residents to and from meals and activity time. iiUniversal workers are responsible for observing and reporting changes in residentsi physical and emotional conditions to the Charge Nurse.i Excellent communication and team skills are needed.i May be required to work a variety of shifts depending on tenant needs.i DUTIES AND RESPONSIBILITIES:iProvides services to tenants as listed on the service plan and as delegated by the Charge Nurse.i iPerform or assist tenants with personal grooming needs such as shaving, hair care, oral hygiene, denture care, skin care and cosmetics. iAssist tenants in dressing and undressing as needed in clothing appropriate to the time of day and season of the year.iAssist tenant in caring for and securing personal articles, labeling clothing, etc.iChange bed linen.iAssist tenants with before and after meal routines such as washing face and hands, transporting to and from the dining area or preparing environment for in room dining. iProvide tenants prompt mealtime assistance if needed.iReport appetite changes and request substitutions when necessary.iPass water and snacks as instructed.iProvides light housekeeping and laundry, remove trash from rooms.iSock clean linens and other items needed for the residents in both supply closets and rooms as instructed.iAssists and helps direct group and individualized activities for tenants as assigned.i iiResponds to tenant emergency calls while on duty and summons help as indicated according to facility policy and procedure.i iAbility to reasonably conform to all rules and policies of this facility.iReports to and communicates with the Charge Nurse about changes in tenant condition or requests or needs for services.i iOther duties as assigned.i PHYSICAL ACTIVITY REQUIREMENTS: Lifting Requirements: The Universal Workers will frequently be called upon to lift various items between 10 and 50 pounds.i Universal Workers may occasionally be required to lift over 50 pounds while transferring a non-ambulatory resident weighing between 100 and 160 pounds.i Carrying Requirements: The Universal Workers will frequently be called upon to lift various items between 10 and 50 pounds.i Universal Workers may occasionally be required to lift over 50 pounds while transferring a non-ambulatory tenant weighing between 100 and 160 pounds.i Sight Requirements: The Universal Workers is required to have vision corrected to average to assess and respond to resident needs.i Specific vision abilities required by this job include close vision, depth perception and ability to adjust focus.
