Job Summary / Purpose

The Virtual Nurse will develop the plan of care and provide comprehensive oversight of the care of the patient remotely on an acute care, medical-surgical unit. The virtual model will include a Virtual Command Center (VCC) where the Virtual Care Nurse will anticipate and direct the care of the patient through evaluating health promotion, risk reduction and clinical decision making. The Nurse assumes accountability for client care outcomes through the assimilation and application of evidence based practice to design, implement, and evaluate client plans of care in a virtual environment setting. The Virtual Care Nurse will design, implement, and evaluate client care by coordinating, delegating and supervising the care provided by the unit based health care team, including registered nurses, licensed practical nurses, certified assistants, hospitalists, specialty physicians, social workers, case managers and allied health professionals. A high degree of professional competence and autonomy will be demonstrated in performing the roles of 1) clinician, 2) educator, 3) consultant/collaborator, 4) leader.

Essential Key Job Responsibilities

  1. Collaborates virtually with physicians, nurses and other health care team members in the provision of clinical care for the patient.

  2. Assesses virtually the medical, physical, and psychological status of patients by reviewing patient records, obtaining a comprehensive problem-oriented health history and monitoring and evaluating laboratory, x-ray, EKG, and other diagnostic studies as medically indicated.

  3. Interprets available data in order to establish a primary plan of care. The plan is developed through an interdisciplinary team approach and through multi-dimensional patient care conference, ongoing patient assessments, and re-evaluation of patient problems or needs.

  4. Prioritizes clinical information accurately and in a timely manner.

  5. Responds to routine, urgent and emergent patient problems appropriately by demonstrating effective decision-making and provides follow-up to ensure that patient problems are resolved.

  6. Works to prevent patient problems from occurring by: direct observation via telemedicine technology, proactive surveillance of adverse physiological trends, and assurance of appropriate prophylactic strategies, medical chart review, and appropriate reaction to patient alerts. Risk reduction will be done through participation in root cause analysis (RCA’s), quality surveillance, as well as, compiling and interpreting large data sets of patient information.

  7. Effectively communicates with members of the bedside interdisciplinary team in a collegial manner to provide safe and error-free care during routine and emergent conditions. Provides courteous and enthusiastic phone communication to employees at hospital sites.

  8. Serves as an advocate on behalf of patients and their families in regards to their cultural, spiritual and care needs.

  9. Prioritizes and effectively handles multiple electronic devices and software applications in a professional and courteous manner to include the ability to manage data and create reports electronically.

  10. Coaches and mentors nurses and nursing support on the floor around the care of the patient.

  11. Virtually educates the patient and families on disease process, procedures and self-care to maximize the level of function and wellness upon discharge.

  12. Virtually assesses and reassesses patients as appropriate.

  13. Focuses on exceeding national benchmarks for quality care delivery and resource utilization for cost containment and length of stay indicators.

  14. Maintains patient confidentiality.

  15. Assumes responsibility for personal and professional growth through participation in unit meetings, in-service programs, professional organization, writing for publication, continuing education programs, and fulfills yearly requirements as stated by policy.

  16. Participates in achieving nursing division goals and supports a shared leadership structure. Participates in achieving organizational goals.

  17. Assists in the orientation of new staff and participates in unit Quality Improvement activities.

  18. Assures appropriate level of understanding, awareness and compliance with all applicable TJC, federal, state and agency laws, regulations, guidelines and professional standards.

  19. Through a defined reporting process, reports clinical, administrative, or technical problems that result in medical errors, unsafe care, or sub-optimal communication between appropriate health care professionals.

  20. Participates in and initiates safety, security and infection control programs that are mandatory as well as those required and provided by the department.

  21. Demonstrates work practices consistent with Catholic Health Initiatives and department-specific safety, security and infection control policies.

  22. Authorized to make entries in the medical record, consistent with and limited to the scope of the job duties and any applicable licensure.

  23. Facilitates a collaborative spirit assuring teamwork by providing direction to staff.

  24. Establishes a relationship with care team to help coach, mentor, and set expectations as needed.

  25. Monitors and provides feedback on performance of employees as requested by Nurse Director.

  26. May utilize delegation skills, but maintains accountability for identifying patient information to ensure improvement of care and patient outcomes.

Marginal Job Functions:

  1. Assumes administrative responsibilities when requested.

  2. Assumes project management as delegated.

  3. Participates in appropriate community activities.

Qualifications

Required Education for Staff Job Levels

  1. Possession of a current or compact state license as a registered nurse as issued or defined by the State of Iowa.

  2. Bachelor of Science in Nursing Degree required or actively in pursuit of BSN degree.

  3. Master’s Degree in Nursing as Clinical Nurse Leader (CNL) preferred.

  4. Master’s Degree in Nursing as Clinical Nurse Specialist (CNS) accepted.

Required Licensure and Certifications

Possession of a current or compact state license as a Registered Nurse issued/defined by the State of Iowa.

Proof of completion of Mandatory Reporter – Child and/or Dependent Adult Abuse training within three (3) months of hire.

Basic Life Support certified within six (6) weeks of hire. Acceptable credentialing bodies and certifications include American Red Cross CPR/AED for the Professional Rescuer and American Heart Association Basic Life Support for Healthcare Providers.

Required Minimum Knowledge, Skills and Abilities

  1. Minimum of five (5) years in clinical focus area preferred.

  2. One (1) year leadership experience) required.

  3. Outcomes management experience required.

  4. High function computer technical skills required.
