GENERAL SUMMARY:

The

Social Worker provides psychosocial assessment, diagnosis of mental health

disorders and participates in treatment planning within a multi-disciplinary

team in partnership with the client or patient.

Provides individual, group and family therapy for assigned clients. Maintains age-specific competencies for

assigned patient/client population.

ESSENTIAL FUNCTIONS:

  • Completes psychosocial assessments and diagnosis of mentalhealth and substance use disorders as per licensure and/or credentials withinprescribed time frames.

  • Provides patients/clients access to patients/client forappropriate community resources to meet needs identified in the assessmentprocess.

  • Facilitates individual, group and family therapy in theoutpatient setting and as part of the active treatment schedule for theinpatient programs. Modifies treatmentplan as needed to meet changes in patient need.

  • Facilitates individual, group and family therapy for substanceuse disorders in the outpatient setting and as part of the active treatmentschedule for the inpatient programs. Modifies treatment plan as needed to meetchanges in patient need per licensure and/or credentials.

  • Provides additional substance use disorder assessment andintervention based on client needs per licensure and/orcredentials.

  • Leads the multi-disciplinary team to develop individualizedtreatment plans based on assessed needs of the patient/client. The treatment plans are developed in partnership with the client/patient and familyto reflect their preferences, strengths and weaknesses.

  • Demonstratesappropriate and effective written, verbal and non-verbal communication withpatients and their families, medical staff, colleagues and the public.

  • Documentspatient/client’s progress per policy.

  • Leads discharge planning.Utilizes family and community resources to assist transition ofcare.

  • Stays current with evidence based practice and implements in treatment.

  • Consultswith multi-disciplinary team to manage a patient/client crisis or emergencysituation.

  • Documentsclinical information consistent with Mercy Behavioral Health standards.

  • Maintains positive working relationships with peers.

  • Accesses clinical supervision for continued learning and skilldevelopment.

  • Maintains high standards of professional accountability regardingsafety, efficiency, efficacy, ethics and legal issues. Maintains patient confidentiality in allaspects of treatment.

Qualifications

MINIMUM KNOWLEDGE,

SKILLS AND ABILITIES REQUIRED:

  • Masters degree in social work or related field and/or CADCis required.

  • Must be license eligible (i.e., social work, mental health counseling,marriage and family therapy).

  • Twoyears experience in mental health facility is preferred.

  • Proof of completion of Mandatory Reporter abusetraining specific to population served within three (3) months of hire

  • Understandsmedical terminology, Medicare criteria and child/dependent adult laws andregulations.

  • Demonstratescompetence in providing advanced social work/therapy services includingpsychosocial counseling with individuals and families.

  • Knowledgeregarding the principles, ethics, theories and methods of therapy.

  • Demonstratesskills in conducting clinical interviews and communicates effectively withpersons of diverse backgrounds and diverse ages.

  • Knowledgeregarding the principles, ethics, theories and methods of therapy.

  • Demonstratesskill in conducting clinical interviews.

  • ValidIowa driver’s license preferred.

  • CDAC or CDAC eligible preferred.
