Social Worker - MSW

FTE: 1.0 (Full-time, 40 hours per week)

  • Maintains the standards of social work excellence in accordance with the scope of practice and standards of care set forth by the proper state chapters of social work and NASW Standards for Social Work Practice in a Health Care Setting

  • Ensures quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.

  • Consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in assigned service area.

  • Conducts assessments, diagnosis, and treatment of the social and emotional factors related to the patients/clients illness, need for care, response, and adjustment to care and completes appropriate plan of care and interventions according to scope of practice.

  • Assesses the relationship of the patient’s medical and nursing requirements to the patient’s home situation, financial resources and available of community resources. Providing information and appropriate referral action to obtain available resources to assist in resolving patient’s problem.

  • Provides counseling/short term therapy that is required by the patient and medical social services to the caregiver/family member according to scope of practice and Home Care Conditions of Participation

  • Assists patient in establishing patient-centered goals of care identifying psychosocial and behavioral strengths and weaknesses

  • Identifies barriers and impediments to patient-centered plan of care

  • Participates in the development of the plan of care and attends interdisciplinary team meetings.

  • Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying.

  • Participates in discharge planning when needed serving as a liaison for transition of care

  • Assists patient/family/caregiver with securing durable power of attorney, living will, and IPOST.

  • Attends staff meetings, IDT meetings, and other meetings as assigned and appropriate.

  • Participates in in-service programs.

  • Serves as a Social Work Intern Preceptor.

  • Maintains accurate and timely documentation of clinical records.

  • Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines.

  • Maintain compliance with Personnel policies and procedures.

  • Maintain regular and consistent attendance at work.

  • Behave in a manner consistent with all Corporate Compliance policies and procedures.

  • Perform other duties as requested by Clinical Manager to facilitate the smooth and effective operations of the organization.

  • Meets measures as required or determined by job productivity and performance improvement.

  • Participates in orientation and training of new employees or social work intern.

  • Achieve certification in Integrated Care Management within 1 year of hire. Certification includes completing the Integrated Care Management formal training session, successfully completing assigned computer based-learning modules within 2 weeks of training, and a competency evaluation. Use the Integrated Care Management philosophy, skills, tools and documentation in clinical practice. Demonstrate competency in selected ICM skills annually.

  • Employee is responsible for completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing videotapes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UPAH programs and initiatives.

QUALIFICATIONS REQUIRED:

  • Master’s degree in social work from a University/College accredited by the council on Social Work Education and State Licensure per State Code.

  • Valid Mandatory Reporter course completion by state(s) requirement

  • CPR Certification.

  • One year clinical experience in health care.

  • Strong verbal and written communication skills.

  • Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.

  • Ability to work as a collaborative team member.

  • Ability to understand and apply guidelines, policies and procedures.

  • Valid licensed driver with automobile insurance in accordance with state and/or organizational requirements.

CONTINUING EDUCATION REQUIREMENTS

All staff are expected to participate in appropriate continuing education. You are required to complete:

  • Mandatory Reporter: Child & Dependent Adult Abuse

  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

In addition, staff are expected to accept personal responsibility for other educational activities to

enhance job-related skills and abilities. All staff must attend mandatory educational programs.
