VINTAGE PARK APARTMENTSVINTAGE PARK IN-HOME SERVICESUNIVERSAL WORKER/HOME HEALTH AIDEPOSITION DESCRIPTIONOBJECTIVE:i Assist nursing personnel in provision of basic care for residents/clients including tasks such as activities for daily living, personal cares and medication administration as required by resident Individual Service Plans and according to the applicable standards and regulations in the State of Iowa, Medicaid, Medicare or Private Insurance providers. .ORGANIZATION:The Universal Worker/Home Health Aide reports directly to the supervising Nurse (LPN or RN /Health Care Coordinator), and indirectly to the Vintage Park Manager and Owners.QUALIFICATIONS:i iBe at least 18 years oldPass a pre-employment drug/alcohol screenPass all required criminal history, background, abuse registry, reference checks, and any/all other required pre-employment evaluations Be able to read, write, understand and carry out directions/instructionsBe able to work independently and as a part of a teamBe able to physically perform job-related duties which require lifting (at least 50 pounds), standing, bending, transferring, stooping, stretching, walking, pushing and pulling, and use job related equipment including an IPAD. Be able to provide visual and auditory observation of residents/clientsPossess the ability to communicate with co-workers and residents/clients effectivelyBe compassionate, sympathetic and professional at all timesBe organized, able to perform varied and diverse job dutiesBe able to remain calm during stressful and sometimes chaotic situationsBe willing and able to work days, evenings, holidays, overnights, weekends Be willing and able to work on short notice, from time to time, as staffing needs require, or to stay beyond the scheduled shift if necessaryPossess a validi chauffeurs license i if transporting residentsPOSITION RESPONSIBILITIES:* iiiiiiii Assist residents/clients with bathing, skin care, toileting, grooming, oral hygiene, dressing and undressing, etc., as required*iiiiiiiii Assist with resident/ client activities as needed*iiiiiiiii Perform medication administration and complete associated documentation as required by state regulation and company policy* iiiiiiii Assist with preparing/serving residents/clientsi meals and cleaning up the kitchen and dining areas after meals*iiiiiiiii Utilize proper transfer techniques and assist resident/client with ambulation as needed, and demonstrate safe use of medical equipmentiiiiiiiiiii *iiiiiiiii Keep residents/clients living space and common areas clean at all timesiiiiiiiiiii *iiiiiiiii Do residentis laundry as per schedule and training protocolsiiiiiiiiiii *iiiiiiiii Make and change bed linens as needediiiiiiiiiii *iiiiiiiii Transport residents/clients as needediiiiiiiiiii *iiiiiiiii Answer resident pendants promptlyiiiiiiiiiii *iiiiiiiii Respond to door alarms promptlyiiiiiiiiiii *iiiiiiiii Measure and record vital signs and weights as directed*iiiiiiiii Report any changes in residents/clients health status to Health Care Coordinator immediately* iiiiiiii Maintain client confidentiality as per regulatory standards and company policy*iiiiiiiii Attend mandatory in-service trainings and staff development opportunities*iiiiiiiii Complete all required documentation in a neat, timely and accurate manneriincluding incident reports, nurse communications, EMARS, Tasks, Housekeeping, Laundry, CDAC, etc.*iiiiiiiii Maintain a clean, neat and professional appearance at all times*iiiiiiiii Work cooperatively with your supervisor and co-workers at all times*iiiiiiiii Extend courtesy and respect to residents/clients, their family members and guests, and Vintage Park visitors, at all times*iiiiiiiii Read and be familiar with resident/client Individual Service Plans*iiiiiiiii Read and comply with Vintage Park Personnel Policies and Vintage Park Policy and Procedure Manual.*iiiiiiiii All other duties as required or assigned To perform this job successfully, an individual must be able
