Henry County Health Center RN - Care Coordinator, Rural Health Clinics in Mount Pleasant, Iowa
Perform selected nursing and care coordination duties that assist in patient care management. These duties are delegated in relation to the degree of training in accordance with the objectives and policies of the organization and respective state laws governing such action and activities.
The Registered Nurse-Care Coordinator will lead and organize all care team members and Preventative and Chronic Care Management program activities. He/She will implement process that follows the patient during care transitions, and improves the communication between all healthcare providers involved.
Communication with treating facilities/providers will allow care team members to collect the patient’s medical information, medication changes, and any discharge planning. Part of this process also needs to include the use of a Continuity of Care Document (CCD). The Registered Nurse – Care Coordinator will work with closely with insurance providers and assist in coordinating patient centered care; maintain patient files, records and other information; may maintain a variety of general clerical records and general department activities; may compile and condense technical and statistical data for use in reports in records; and participates in additional activities is indicated by department and or clinic needs.
Depending upon the location and staffing, the Registered Nurse performs designated nursing duties as delegated by an advance registered nurse practitioner, and/or physician. These duties include but are not limited to preparing patient for examination and/or treatment: assisting the provider as directed; taking patient history and vital signs; setting up exam/treatment rooms and instruments for minor surgical procedures; applying dressings; preparing and maintaining supplies and equipment used for treatments administered, including necessary sterilization and observation of the septic techniques. The Registered Nurse assists the provider in preparation for minor surgeries, physicals, giving injections and applying splints.
The Nurse may also be responsible for a variety of general office clerical duties that include: arranging provider schedule, setting a patient appointments and/or referrals, answering and directing telephone calls, providing general information on department policies and procedures, composing and typing general correspondence, ordering and maintaining both clinical and office supplies and equipment. RN must also have functional knowledge of the computer system to register patients and enter charges within the system.
The Registered Nurse must show a knowledge of common safety hazards and precautions to establish a safe working environment. Must demonstrate skill in establishing and maintaining effective working relationships with patient's medical staff and the public. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly.
The Registered Nurse will participate in those activities of which the RN licensure allows and with appropriate orders and protocols established by the Physician and/or ARNP.
The Registered Nurse will participate in a variety of professional activities to keep abreast of current changes and trends in the healthcare industry.
Associate in this position must demonstrate the ability to: Competence in the field requires that a Registered Nurse display professionalism in all aspects of appropriate nursing and human relations skills, communicate effectively and provide the necessary instruction to patients for optimal physical and psychosocial well-being.
Education and experience
Graduate with a minimum of an Associate Degree in Nursing from an accredited registered nurse program.
Annual requirements, licensure, and certification
Must hold active State of Iowa RN License
Machines, tools, equipment and work aids; Competence in operating typical clinic equipment
