POSITION SUMMARY: The RN Case Manager works with the Care Coordination team to facilitate patient care during their hospital stay. The Case Manager will work closely with the providers to ensure proper care for a safe transition at discharge.


  • Assists Utilization Review and negotiates lengths ofstay and transitional plans with relevant others.

  • Completes a comprehensive assessment of the client’sholistic health status and needs to include clinical condition, supportsystems and resources, client/family knowledge, perception and adjustmenttowards disease process, and community resources.

  • Screens patients/clients for identified and potentialneeds. Makes referral toappropriate multi-disciplinary services.

  • Identifies high-risk/complex patients for ongoing casemanagement.

  • Interacts with patients, families, physicians, andother health team members to develop proactive plans for continued care.

  • Evaluates and modifies plans according to changingpatient needs and clinical data; documents and communicates plan to allteam members.

  • Intervenes when medical complexity results infragmented care.

  • Identifies and acts to eliminate gaps in clinicalcare; makes referrals in collaboration with discharge planning asindicated.

  • Facilitates the multidisciplinary team rounds toidentify patient’s needs up discharge and to establish an effective planof care to meet the patient’s continuum of care needs.

  • Rounds and works closely with providers to ensureappropriate care is being provided.

  • Completes discharge phone calls to patients dischargedhome.

  • Consults clinicians, other departments, and committeesas needed to support the patient through the care continuum.

  • Collaborates with appropriate staff and management tofacilitate the development of case management tools, i.e. pathways andprotocols, clinical indicators that demonstrate best practice and diseasestate management processes.

  • Swing bed Activities program coordinator to ensureguidelines are being met set by CMS.

  • Adheres to standards of care set bythe department.

  • Demonstrates professional judgmentconcerning patient care delivery

  • Demonstrates ability to establishpriorities and organize effectively.

  • Assumes responsibility forprofessional growth and continuing education.

  • Attends 85% of departmentmeetings/in-services.

  • Demonstrates professional writtenand verbal skills.

  • Demonstrates professional attitudeand interest in work by maintaining acceptable work habits and behavior.

  • Assumes advocate role for patientsas a liaison between the patient/family and physicians, the hospital,insurers, community resources and others.

  • Demonstrates ability to maintainprofessional demeanor in stressful situations.

  • Demonstrates commitment to customerservice.

  • Demonstrates ability to workeffectively in a multi-disciplinary team.

  • Demonstrates ability to communicateprofessionally and assertively with a variety of customers including butnot limited to, physicians, patients and families, co-workers andinsurers.

EXPERIENCE: Minimum of 5 years clinical nursing in

a hospital setting. Minimum of 2 years’

experience in case management or demonstrates competency with commensurate



Current license as a Registered Nurse from the State of Iowa required,

BSN preferred.

QUALIFICATIONS: Must be able to read,

write and follow written and oral instructions in English. In addition, must

be: able to operate computer efficiently; skilled in meeting the growth and

development needs of infant, child, adolescent, adult and geriatric patient

groups; and must possess knowledge and understanding of cognitive, physical,

emotional, and chronological maturation processes of these patient groups. Case

Management certification preferred. Proof of completion of Mandatory Reporter

abuse training specific to population served within three (3) months of hire.
