Drug Screen RequiredPURPOSE OF THIS POSITIONThe registered nurse is responsible to provide relief and on-call nursing services to Care Initiatives Hospice patients in coordination with the RN case manager. Responsible to oversee licensed practical nurses assigned to Care Initiatives Hospice patients; responsible to establish and manage hospice aide/hospice homemaker services provided to patients/families/caregivers. Functions according to operational policies and procedures to meet patient/family/caregiver needs in compliance with federal, state and local requirements, Care Initiatives Hospice Policies and Procedures, and Care Initiatives Hospice Clinical Team Procedures.ESSENTIAL FUNCTIONSProvides nursing visits as directed by the RN case manager.Responds to emergency/on-call needs of patients/families/caregivers.Responds to referral/admission requests from patients/families/caregivers; able to discuss and explain Care Initiatives Hospice care to patients/families/caregivers referred for careObtains informed consent and initiates admission process.Completes initial nursing assessment/evaluation of patients.Qualifications: Education/Certification/LicenseRegistered nursing education.Current, valid Iowa registered nurse license.Current Iowa driver's license and automobile liability insurance.Current, valid CPR certification.Obtains Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment (required for full and part-time staff only; optional for casual employees).Current home access to the internet.KnowledgeKnowledge of care of the terminally ill.ExperiencePrefer two years recent clinical experience in medical/surgical, oncology, home health or public health nursing.Skills/AbilitiesDemonstrated tactful, supportive and sensitive approach to patient/family/caregiver needs.Ability to plan, organize and utilize patient/family teaching principles regarding health care needs.Ability to exercise independent judgment, plan and organize.Ability to effectively communicate both orally and in writing.Ability to access businesses and patient homes.Ability to provide lifting, turning and ambulatory assistance to patients.EOE / AAP Veterans and Disabled
