Job Description
This position is located in Woodward, Iowa.
Under general supervision, will assist in planning and implementing Individual programming in interdisciplinary teams.
During activity periods, provide direct training to Individuals and assist RTW's in conducting specific training activities as determined by the IDT. Monitor Individual activities, assist in developing training activities relevant to Individuals ELP's and Individual program plans. Participate in Individuals ELP process and assist in structuring and conducting activities as developed by the IDT.
Prepare, order, and maintain activity materials, schedules, scenarios, etc., as needed to complete Individual activities. Communicate with TPM's, direct care staff, supervisors, activity specialists, in order to facilitate client ELP needs. Adapt materials equipment as needed per clients specific needs
Assist in supporting and carrying out dining issues for assigned Individuals as determined by the IDT and TPM. Complete specific functions of dining scenarios including assisting with feeding, performing "running issues" and holding activities as needed.
Assist in completing Individual self and activities of daily living functions as needed for assigned Individuals utilizing individual program plans, partial participation and accepted practices of "active treatment".
Complete, monitor, and maintain activity and Individual records as needed. Complete the clerical type functions related to Individual activities and staff documentation requirements, etc. (i.e. work order logs, program training sheets, data transposing, etc.) Complete related duties as assigned.

Additional Qualification Requirements
Designated positions in this job class require applicants to obtain the required Commercial Drivers License and endorsement within a period of time as determined by the appointing authority at the time of hire.
